I hereby agree/authorize my child to participate in this soccer training program being  offered by VAST Soccer Training, LLC. I understand that my/my child’s participation in this program is completely voluntary, and I have familiarized myself with the activities which are part of the program. I understand that there is a risk of injury by participating in this program and I agree to assume the risks and/or allow my child to assume the risk of injury, even if arising from the negligence of VAST Soccer Training, LLC or others.

In consideration of VAST Soccer Training, LLC permitting me/my child to participate in any way in any event held at or affiliated with VAST Soccer Training, LLC, I hereby acknowledge, agree and represent that I understand the nature of the soccer training program and that I am/my child is qualified, in good health, and in proper physical condition to participate in the program. I further acknowledge and agree that if I believe my child, myself, or a member of my household is infected or at risk of contracting or spreading virus or bacterial agents (including, but not limited to COVID-19), neither myself nor my child will enter the VAST Soccer Training, LLC facilities and I am solely responsible for my/my child’s actions in this regard.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to me/my child (including, but not limited to personal injury, disability, and/or death), as well as any illness, damage, loss, claim, liability, or expense of any kind that

I/my child may experience or incur in connection with VAST Soccer Training, LLC, its activities, and/or it’s facilities. I HEREBY RELEASE, COVENANT NOT TO SUE, FOREVER DISCHARGE AND HOLD HARMLESS VAST Soccer Training, LLC and its facilities, owners, coaches, contractors, agents, and representatives, of and from any and all claims, including all liabilities, claims, actions, damages, costs, or expense of any kind, including any and all claims relating to exposure or contraction of illness or  injury related to viral or bacterial agents (including, but not limited to COVID-19) 

I fully understand that:

(A) This program involves risks and dangers of serious bodily injury, including permanent disability, paralysis and death (“Risks”);

(B) these Risks may be caused by my/my child’s actions or inactions, the actions or inactions of others participating in the program, the actions, inactions, or negligence of VAST Soccer Training, LLC and its agents, as well as the conditions in which the program takes place;

(C)there may be other risks or social and economic losses either not known to me or not readily foreseeable at this time; and

(D)by executing this waiver and release, I fully accept and assume all such risks and all responsibility for losses, costs, and damages I/my child incurs as a result of my participation, or that of the minor, in the program.

In consideration of VAST Soccer Training, LLC accepting and permitting me/my child to  participate in this program, I understand and agree that VAST Soccer Training, LLC and  each of its facilities, members, principals, employees, agents, and volunteers will not be 

liable for any injury, death or damage, including property damage, to me/my child or  anyone claiming on my/my child’s behalf. I further agree to hold harmless, indemnify and defend VAST Soccer Training, LLC and each of its facilities, members, principals,  employees, agents, and volunteers for and from any and all liability, claims, losses,  injuries, illness, or death to me/my child, or property damage during the time of my/my  child’s participation in this program.  

A parent or a legal guardian or participant (18 years or older) must check below  acknowledging he/she has read, understands, and agrees to all of the above.
